At Horizon Healthcare Services, our infusion therapy services involve more than just compounding and delivering intravenous medications. Clinical pharmacy excellence serves as the basis for a range of therapies provided to treat a variety of disease states. Horizon's range of services includes 24-hour availability of pharmacists and specially trained home infusion nurses, care coordination, infusion compounding, infusion equipment and supplies, delivery, billing, patient and family education programs, flexible, customized treatment programs, skilled reimbursement specialists, and a rigorous performance improvement program. HHS also provides ongoing monitoring and review of treatment plans and program modification per physician caregiver at critical points in the treatment plan.

Experienced pharmacists compound therapies at our Horizon Healthcare Services pharmacy located in Lancaster, PA. Based on consultation with the referring physicians, our pharmacists and the patient's home infusion nurse develop a patient and family education program to achieve a level of self-care appropriate for each individual patient. The team continues to monitor patient compliance and response to treatment, offering 24 hour access to Horizon Healthcare Services' pharmacists and nurses.
